The secret to this little puzzle from Amica is that they will not mail you a US auto insurance card. I guess they don't want to tell you that directly, so they have this passive-aggressive form that sort of indicates you maybe? can't select the "Include Domestic card" option even though they provide it. Then, they give you an error if you try to submit the form with it.
